Man crushed to death at Top Mix Cement, ministry probing

The Occupational Safety and Health (OSH) Department of the Ministry of Labour has launched an investigation into a fatal workplace incident involving  Patrick Picket, a serviceman at Top Mix Cement, located at Lot 725 Block, Great Diamond, East Bank Demerara.

 

A release from the OSHA today said that the investigation follows a report detailing that on Saturday, October 12, 2024, at approximately 10:45 a.m., Picket sustained fatal injuries from the rear right wheels of a 14-tonne ready-mix cement/concrete truck, operated by Sham Singh. At the time of the accident, Picket was servicing the vehicle, checking the differential oil underneath, when Singh drove the truck to the batching plant for loading.

 

Picket was found lying injured on the ground by Harry Mahase, Batch Supervisor, and other staff members. Mahase immediately transported him to Diamond Regional Hospital, where he was pronounced dead, the release said.

 

OSH Officer Neza King conducted an initial site visit to begin the investigation and gather information about the circumstances surrounding the accident. Unfortunately, primary witnesses were unavailable at the time, as they were providing statements at the police station.
